What is the Process for Alliant Rehab Insurance Coverage Approval?

When you call the Ingrained admission representative for assistance with insurance pre-approval, we will need to ask for details to streamline the process and avoid unnecessary delays. We ask that you call with the following information ready:

• Your insurance plan ID number and member details
• A copy of your insurance plan card, as well as any co-insurance or secondary coverage available
• Current contact information
• Date of birth
• The name and contact details of the insurance provider, especially if it differs from what appears on your card (this can happen during a company name change)
• Basic details about your substance abuse history and any recent relapses or withdrawal symptoms
• Information regarding any prior treatment, including hospitalizations or outpatient treatment
• A list of current prescription medications
• Details on your current living situation, including whether housing assistance or supportive housing may be needed during or after treatment
